Fee Schedule Florida Administrative Code Section 7.00 Description of fees:
(1) Employees covered by a health plan that is subject to federal health care law must pay a fee when remitting or receiving their employment-related compensation to the health insurance carrier under the plan and remitting or receiving information to the carrier in connection with the plan.
(2) Employers covered by a health plan that is not subject to Federal or State law which requires an individual to pay for the health care coverage provided by the employer, shall pay a fee when filing or receiving their employment-related compensation of such an employee. A fee is not required for the employees covered by more than one health plan.
(3) Employers required to make the remittance required under subsections (1) or (2) shall pay a fee when remitting or receiving the remittance at the time of filing or receiving an employment-related compensation.
(4) A fee collected pursuant to this section shall be forwarded to the Department of Labor.
Section 7.01.
Fee for Filing or Receiving Information; Amount of Filing Fee.
(1) Any person (other than a state or local agency) required by Section 6.11 to file the remittance required by subsection (4) or to require the remittance specified above shall pay for filing the information as specified above as follows:

Any person making payment pursuant to this section shall include a receipt for payment to the State of Florida. A fee shall be assessed for any failure to remit remittance or provide information as required under this section. (2) Any person (other than a state or local agency) required by Section 6.11 to file or provide information shall pay for receiving the information as specified above as follows:

The amount for remittance shall be the amount of remittance due. (3) The state shall charge the fee received pursuant to subsection (1) or (2), or the fee for providing the information as specified in subsection (1) or (2), according to the rates imposed by law, whichever is greater.
